how long does adderall stay in your system blood work - laboratory testing equipment

Short answer: Adderall (amphetamine salts) usually shows up in a blood test for roughly 12 to 46 hours after the last dose, and most people are at the low end of detection by about day two. Blood has the shortest window of the common test types. The longer story depends on your dose, whether it's IR or XR, how your body handles amphetamine, and, oddly enough, how acidic your urine is. Let's walk through it plainly.

The quick numbers

SampleTypical window after last doseNotes
Blood (serum or plasma)About 12-46 hoursShortest window; shows recent use
SalivaAbout 20-50 hoursCommon in roadside and some workplace screens
UrineAbout 2-4 daysThe most common workplace sample
HairUp to about 90 daysShows a pattern of use, not last week

These ranges come from published pharmacology references and lab guidance, and they are averages. Real people fall outside them in both directions. For the full set of windows for other substances, see our detection windows table.

Why blood clears so fast

Blood tests look for the drug itself (amphetamine) circulating in your bloodstream right now. Once the liver and kidneys pull it out, the blood level drops below what the lab can measure, even though traces are still on their way out through urine. That's why blood is the go-to sample when someone wants to know "was this person affected recently," like after a crash or in an ER, and why it's a poor tool for spotting use from last week.

Amphetamine's elimination half-life in adults is commonly listed at around 9 to 14 hours. One half-life means about half of the drug is gone. It takes roughly four to five half-lives for a drug to fall to very low levels, which lines up with that roughly two-day blood window.

Is Adderall even on routine blood work?

This is the part a lot of people miss. A standard blood draw at your physical, like a CBC, a metabolic panel, cholesterol or A1C, does not test for drugs at all. Those tests measure blood cells, electrolytes, kidney and liver function, sugar and fats. A lab only runs a drug screen on blood if a provider specifically orders one.

When blood drug testing does happen, it's usually because of:

So if you're worried your annual bloodwork will "show Adderall," the honest answer is that it almost never looks. If you're unsure what was ordered, ask the clinic. They will tell you.

What changes your personal blood window

Immediate release vs extended release

IR tablets peak in blood in about 3 hours. XR capsules release in two waves and peak closer to 7 hours, so the drug lingers in your blood a bit later into the day. That can push the end of the blood window out by several hours compared with the same total dose of IR.

Dose and how long you've taken it

A single 10 mg tablet clears faster than 30 mg daily for months. Higher doses take more half-lives to drop under the lab's cutoff. Daily use doesn't make amphetamine pile up in fat the way THC does, but steady dosing keeps a baseline level in your blood.

Urine pH, the sleeper factor

This surprises people. Amphetamine leaves the body faster when urine is acidic and much slower when it's alkaline. Studies have shown the half-life can swing from roughly 7 hours with acidic urine to well over a day with very alkaline urine. Diet, antacids and some medications nudge urine pH. That's why two people on the same dose can have very different clearance times.

Age, weight and kidney function

Kids clear amphetamine a little faster than adults. Reduced kidney function slows it down. Body weight matters less than most people think for amphetamine, since it doesn't store in fat.

Other medications

Some antidepressants, acid reducers and other drugs interact with how amphetamine is processed or excreted. If you take anything else regularly, your prescriber or pharmacist can tell you whether it affects your levels.

Blood vs urine: which would catch it?

Urine wins on time. Amphetamine and its metabolites stay in urine for roughly 2 to 4 days in most people, sometimes a bit longer with heavy, frequent use. Blood is the narrow window. If a test is a pre-employment screen, it is almost certainly urine or saliva, not blood. Our page on urine vs blood windows for alcohol explains the same basic difference with another substance.

Cutoff levels for amphetamine

Labs report a result as positive only when the level is above a set cutoff. Common workplace urine cutoffs for amphetamines are 500 ng/mL for the initial screen and 250 ng/mL for confirmation testing, following federal-style guidelines. Blood testing uses much lower levels because concentrations in blood are smaller, and labs set their own reporting limits. You can compare urine cutoffs across drug classes in our cutoff levels table.

Can Adderall cause a false positive on blood work?

It's the other way around in most cases: Adderall is an amphetamine, so it will be a true positive for amphetamines if a drug screen is ordered. Confirmation testing (GC-MS or LC-MS/MS) identifies the exact compound. Separately, some medications can make an initial immunoassay screen flag amphetamines when no amphetamine was taken, including certain decongestants, some antidepressants and a few others. That's why lab confirmation exists.

If you have a prescription

If you take Adderall as prescribed, you have nothing to hide from a properly run test. Here's how it normally works:

  1. The lab confirms the positive and sends it to a Medical Review Officer (MRO), a doctor who reviews results.
  2. The MRO contacts you privately and asks about prescriptions.
  3. You give the pharmacy name, prescriber or a photo of the bottle.
  4. The MRO verifies it and reports the result to the employer as negative. The employer usually never learns what you take.

Keep your prescription current and the bottle handy. Don't take someone else's pills; a borrowed prescription won't verify.

If you don't have a prescription

Using Adderall without a prescription is illegal and carries real health risks, including heart rhythm problems, high blood pressure, sleep loss and, with regular use, dependence. If you've been using it to study or work longer hours and you're finding it hard to stop, that's worth talking about. How to check yourself before a test

A home amphetamine test uses urine, so it will catch use for longer than a blood test would. That makes it a cautious check: if a home urine strip reads negative, a blood test taken at the same time would almost certainly be negative too. A few tips:

Our guide on how to read a home drug test walks through faint lines and invalid results.

A realistic timeline example

Say an adult takes a single 20 mg IR tablet at 8 a.m. on Monday. Blood levels peak around late morning, drop by half roughly every 10 hours, and are usually below most labs' blood reporting levels sometime between Tuesday and early Wednesday. Urine would likely stay positive into Wednesday or Thursday. Change any variable (XR, higher dose, alkaline urine, weeks of daily use) and those times stretch. This is an illustration, not a guarantee, and it's no substitute for asking your prescriber.

What to tell the person drawing your blood

If you're getting blood drawn for a medical reason and you take Adderall, mention it. It matters for anesthesia, for some heart tests and for reading certain lab values. Medical staff aren't there to report you. They need accurate information to keep you safe.

Bottom line

Adderall usually leaves a blood test's detection range within about 12 to 46 hours, faster than urine, saliva or hair. Routine blood work doesn't test for it unless a drug screen is specifically ordered. If you have a prescription, the MRO process protects you. If you don't, the only reliable way to be clear is time without use, and help is a free phone call away.

Questions people ask

Does a CBC or metabolic panel show Adderall?

No. Those tests measure blood cells, electrolytes and organ function. A drug screen has to be ordered separately.

How long does 20 mg of Adderall stay in your blood?

For most adults, roughly a day to a day and a half after a single dose, sometimes up to about 46 hours.

Does Adderall XR stay in blood longer than IR?

A little. XR peaks later in the day, so blood levels stay measurable a few hours longer for the same dose.

Will my employer see my Adderall prescription?

Normally no. The MRO verifies the prescription privately and reports a negative result to the employer.

Does drinking water flush Adderall from blood faster?

Not meaningfully. Hydration doesn't speed up how fast your liver and kidneys clear amphetamine from blood.
